There is an Ohio Bureau of Motor Vehicles exam station located in the plaza with the license bureau. Residents of any county in the State of Ohio may test at this location. Written tests are on a "walk-in" basis. Driving tests are by appointment only.   Appointments can be made online at:

Mayfield Hts-Golden Gate License Bureau is an authorized WebCheck location for the Bureau of Criminal Investigation under the Ohio Attorney General's Office. Many Ohio employers, both public and private, are required to perform background checks on new hires and individuals obtaining professional licenses. The Attorney General's Office's Bureau of Criminal Identification and Investigation (BCI) completes such background checks by comparing fingerprints received against a database of criminal fingerprints to determine if there is a criminal record. All fingerprints must be submitted to BCI through WebCheck or card scan unless exemption reasons are met.

WebChecks are a cash only service.


Mayfield Hts-Golden Gate License Bureau is able to collect reinstatement payments on behalf of the State of Ohio. Should documentation need to be forwarded to reinstatement, customers should be knowledgeable on what is needed and bring those items at time of payment. Those requirements can be found by calling reinstatement at 844-644-6268 or by using the online services section of the BMV website at There is a $10.00 fee for reinstatement payments or faxing of documents.
